INFORMATION
When the death occurs in Hospital.
A member of the nursing staff will speak to you and arrange for a hospital doctor to issue the Medical Certificate of Cause of Death, which you will need to take to the Registrar. We will advise you of the Registrar's telephone number and office hours together with the information the Registrar will require. The hospital staff will enquire if the funeral is to be cremation or burial as the documentation will vary.
When the death occurs at Home.
When the death takes place at home you must first notify the family doctor who will attend to confirm that death has occurred. Once the doctor has attended you should contact us so that the necessary funeral arrangements may be made. The doctor will normally issue the Medical Certificate of Cause of Death, for you to take to the Registrar.
Sudden or unexpected death.
If the death is sudden or unexpected, after the doctor has attended, it is normal for the doctor to notify the Police to attend to collect information. The police are acting on behalf of the Coroner. We will liase with the Coroner's Officer on your behalf, explain the procedures and make the necessary funeral arrangements. We will also advise you when to attend at the Registrar.

Funeral Arrangements.

We, as your Funeral Director who will be helping you with the funeral arrangements, will need to know the full name, age and religion of the deceased. We will also need to know if the deceased's wishes are for a burial or a cremation. In either case we will help you complete the statutory applications.
If a burial is required and there is already a family grave, we will need documents or details of the grave in order to locate the grave. If a new grave is required, you should decide who will have the exclusive right of burial and be named on the deed.
If a cremation is required, we will arrange for two doctors to complete the Doctor's Cremation forms and arrange for them to be delivered to the Crematorium. We will discuss with you the various options available for the cremated remains.
Our Service Chapel of Rest.

Our Chapel of Rest is at 123 North Road, Porth. Visiting is welcome, preferably by prior appointment.
Minister.
We will contact your chosen Minister or Officiant on your behalf. You will need to discuss with your Minister, service arrangements, hymns and music. If you do not have a Minister, we can arrange one on your behalf.
Newspaper Notices.
We will help you to prepare newspaper notices and acknowledgements. We can have these inserted in local or national newspapers.
Funeral Limousines.

Our grey limousines carry 7 passengers. Usually the Minister likes to ride with you in the limousine to Cemetery or Crematorium.
Floral Tributes.
Usually families make their own arrangements for floral tributes but, if requested, we can liase with the florist on your behalf. Flowers may be delivered to your home or to our premises. If we are to receive the tributes, we will list the names in a booklet and present this to you. Cards are not removed off tributes unless specifically requested.
Funeral Stationery.
We can liase on your behalf with a printer to have Service Sheets, Thank You cards etc. printed.
Pre-Paid Funerals.
Pre-Paid Funerals are available with Perfect Assurance Funeral Trust and can be arranged with you.
